24/7 Advice
A unique part of the HSS service is our highly skilled Registered Nurse Case Managers who oversee every part of your or your loved ones care. They are available 24 hours a day, 7 days a week to discuss any queries or concerns you may have.
Our Case Managers also keep in contact with you, whether it’s just a friendly ‘how are you?’ call through to following up after you or your loved one has been visited by our health care staff.
Flexible Care
Our Registered Nurses will be happy to discuss your specific care needs and tailor a package to suit you or your loved ones individual care.
Ongoing communication and monitoring between you or your loved one, your GP and our health care team ensures that the appropriate level of care is in place at all times.

Registered Nurses
- Assess and provide advice on your general health and wellbeing
- Monitor your blood glucose levels and blood pressure
- Help you plan your healthy lifestyle to keep you fit and well
- Offer discreet advice on continence issues
Physiotherapists
- Our Physiotherapists can provide you with an assessment and strategies for maintaining and or restoring physical function.
- Services include:
- Tailoring a personalised conditioning and exercise program
- Offer advice on regaining and or maintaining your strength and balance
Occupational Therapists
Our Occupational Therapists provide holistic and goal orientated care including:
- Assessing the safety of your home to minimise the risk of falls.
- Organising appropriate equipment to assist your mobility and safety in your home
Carers & Domestics
HSS also offers personal care and domestic assistance service to provide that extra help around the house. Services include:
- Showering, dressing, grooming
- Help with meal preparation
- Light domestic duties (washing, ironing, vacuuming)
- Taking you to and from medical appointments and shopping
- Collecting you from hospital and taking you home via the chemist or shops
Care Liaisons
At HSS we understand how confusing and time consuming it can be to wade through the multitude of community services available to find the one that will suit you or your loved one. The forms can be difficult to understand and getting time to ring services can be hard with the pressures of work and family commitments.
We have the skill, expertise and knowledge to do this for you. We can work with you to identify your needs, contact service providers, fill in the forms, check on the progress of applications, provide feedback and updates to you and your family, and keep your GP informed.
Whether it is an ACAT assessment for an increase in services, making an application to a meal service provider, or talking you through your options, we have case managers who can become your ‘Care Liaison’ and provide their years of experience to help you.
We will work with you, your family and other health service providers to help you negotiate your way to the community services you need.

How To Arrange Our Services
At HSS we pride ourselves on providing a premium level of care to our clients. Our complete care management service is one of a kind.
You or your family member can contact HSS to enquire or arrange HSS care by calling the phone number listed below or by clicking on ‘request more information’ and entering the relevant information.
Alternatively, your GP, health professional or medical practitioner can arrange your care with HSS.
You may be eligible to receive a rebate from your Private Health Fund to provide your care. Alternatively you can pay privately. Enquire with HSS who can confirm your health fund eligibility.
