Community / Funded Programs

HSS are providers for a number of government & community funded programs. Via our Care Liaison program, we can work with you, your family and other health service providers to help you find your way to the community services you need or are eligible for. Funded programs that HSS are current providers include:

Department of Veterans' Affairs

DVA Community Nursing and Veterans' Home Care program offers nursing care to eligible Veterans in the comfort of their own home.  Services are flexible and tailored to meet individual needs.
These services include: 

  1. Wound care
  2. Continence management
  3. Palliative care
  4. Medication management
  5. General nursing care
  6. Personal care
  7. Domestic assistance
  8. Respite care
  9. Home & Garden Maintenance

Transition Care Package - QLD

Transitional Care Packages offer an alternative to an extended period of hospitalisation by providing rehabilitation, nursing and support services.  The programs are targeted for older people who are in hospital and need support at home.  Participants are offered an individually tailored program of transitional care for typically 8-12 weeks.  Transition care provides a range of services dependent on individual need including:

  • Nursing support
  • Low intensity rehabilitation
  • Personal care
  • Medical support/GP oversight
  • Case management

EACH-D

Extended Aged Care at Home Dementia (EACHD) packages are individually planned packages of care tailored to help frail older Australians who experience difficulties in their daily life because of behavioural and psychological symptoms associated with dementia.  The types of services that may be provided as a part of an EACHD package include:

  • registered nursing care;
  • care by an allied health professional such as a physiotherapist, podiatrist or other type of allied health care;
  • personal care;
  • transport to appointments;
  • social support;
  • home help; and
  • assistance with oxygen and/or enteral feeding.
