Case Management
The Registered Nurse who visits you in your home works closely with our office based Case Managers who are also are all experienced and highly skilled Registered Nurses. Our Case Managers have the technology at their fingertips to ensure all the information your visiting nurse needs to take care of you is readily available. Case Management ensures that all the aspects of your care are brought together under the care and guidance of one experienced staff member.

Hospital Discharge Support

If you decide you would prefer an early discharge from hospital and your doctor is agreeable, or you feel you need additional support upon discharge we can assist. HSS will work collaboratively with your Medical Specialists to ensure you receive the most appropriate care on your return home. Our nurses are also excellent educators and if you have to learn to take over your own care we can equip you with all the knowledge you need to look after yourself.

Domestic Support
Additional supportive services for a broad range of domestic and family home help (ie cleaning, shopping, laundry, meal preparation, family care activities) can be provided by our team of friendly, experienced carers, allowing you to rest and recover without domestic concerns.

Care Liaisons
If your care becomes a long term requirement, our knowledgable team of Care Liaisons have the resources available to guide you through what can be a complicated system, to determine your eligibility for community programs. This consultancy service can range from simple advice on who to contact, right through to assisting with application and eligibility forms and making contact with relevant community organisations. With over 20 years experience in this area, HSS can ensure a smooth transition of care if and when the time comes.

At HSS we pride ourselves on providing a premium level of care to our clients. Our complete care management service is one of a kind.

You or your family member can contact HSS to enquire or arrange HSS care by calling the phone number listed below. We will then liaise with the doctor on your behalf if required.

Alternatively, your GP, hospital or specialist can arrange your care with HSS. You may be eligible to receive a rebate from your Private Health Fund. Enquire with HSS who can confirm your health fund eligibility.
