What Is Hospital Avoidance?
The Home Support Services (HSS) Hospital Avoidance program has been developed with the fundamental goal of reducing the nights each patient spends in an acute hospital setting. This includes avoiding a hospital admission altogether or enabling patients to leave hospital earlier than would otherwise be possible through thorough case management and the rapid delivery of appropriate services within the patients home.

HSS has a well established 24 hour capability, seven days a week based in South Australia, Queensland and Northern NSW, with rapid response teams of both nursing and allied health professionals. Our systems are designed to quickly deploy the specific services, products and equipment required to safely maintain and treat each patient in their place of residence with dignity and respect for individual choice. We understand that speed of response is essential to avoiding hospital admissions and enabling discharges from hospital to home.
We provide services that range from respite and support of Activities of Daily Living through to complex intravenous therapy, wound care, and rehabilitation services. We have a wide range of clinical specialties such as: Paediatric, Palliative / end of life care, Mental Health, Diabetes, Stomal and Maternity Care.
We deliver care related to maintaining chronic conditions to avoid hospitalisation; enabling patients who have been stabilised in the hospital to quickly return home for the continuation of their treatment and rehabilitation; and avoiding readmissions via a combination of monitoring, treatment, and patient and carer education.
We have also demonstrated our ability to respond quickly to changing needs within the various programs we work within. Most significantly we have proved our ability to respond to the urgency and critical needs of state emergencies such as heatwaves and pandemics.

Home Support Services (HSS)’ business is built on a desire to provide the highest-possible standard of care to people in their own homes or place of residence. HSS pioneered ‘hospital care in the home’ in South Australia and for the past 21 years has worked tirelessly to allow aged and ill people to live with dignity – in the comfort of their own home.

HSS has strived to create a unique all encompassing service for each individual patient that is referred to one of our Hospital Avoidance programs. The HSS Case Management System underpins this service.
HSS has worked with many partners in delivering Hospital Avoidance services including Government Departments, Private Insurance Companies and Individual Hospitals. Each program requires a unique approach and understanding to ensure the particular needs of the patients within these settings are fully met and the objectives of the Health Care partner are achieved.
