• Any person of any age who has been assessed as being at risk of an admission to a metropolitan public hospital or visit to an emergency department, which can be avoided by receiving short term care in their own home or residential facility. • Metropolitan public hospital inpatients of any age that have been identified by hospital staff as suitable for an early supported discharge with a level of care that can be delivered in their own home. |
| Metro Home Link will work closely with doctors, health care workers and hospital staff to ensure we deliver a level of care that can be safely provided in the home. Services can include; • Nursing care (including specialist nursing support in residential facilities) • Specialist paediatric and midwifery services • Allied Health services – such as Occupational Therapy and Physiotherapy • Personal Care – such as assistance with showering and meals • Respite and support for carers • Overnight support if required • Overnight accommodation following a day procedure • Emergency accommodation • Help with some home duties • Transport assistance (only related to your medical treatment) • Short-term supply of medical equipment • Other services depending on your need |

Metro Home Link is a SA Government funded service that is available to provide short term support and assistance for people in their own home or residential care facility.
The aim of the program is to avoid preventable visits or admission to hospital, or to assist patients to leave hospital earlier than would otherwise have been possible in a safe and supportive environment.
