HSS work closely with community health providers, The Department of Veterans Affairs, hospital staff and General Practitioners to ensure that our care of veterans is managed holistically and that all clients receive the very best community care and health outcomes.

Our skilled staff including Registered Nurses, Carers and Occupational Therapists can assess and provided specialist care to Veterans in their own homes.

HSS can also provide domestic services, respite and personal care to veterans via the Veterans Home Care program.

Our Carers Service can offer personal care such as showering, dressing and grooming, mobilisation and toileting assistance.

How To Arrange This Service

All veterans with a Gold Card are eligible for fully funded community health care.  White Care holders may also be eligible with an accepted war or service caused problem requiring health care.

Referrals can be made by a General Practitioner, or hospital discharge planner.
